Bankruptcy Appellate Panel of the Tenth Circuit (BAP)

Declaration Regarding Payment Advice

A form for debtors who do not have employee income records to file. For example, if the debtor is self-employed, retired or unemployed.

Notice to Individual Consumer Debtor

A pre-printed form that explains the different chapters of bankruptcy, the debtors rights and information on credit counseling agencies. This form should be filed with the debtor's petition.

Proof of Claim

Form B10. A form telling the Bankruptcy Court how much the debtor owed a creditor at the time the bankruptcy case was filed (the amount of the creditor's claim). This form must be filed with Clerk of the Bankruptcy Court where the case was filed.

Order form for ordering copies of bankruptcy cases from the National Archives and Records Administration
